Supporting reentry through culture and care

Our Program

Our Justice Involved Program is dedicated to walking alongside community members who are currently or formerly incarcerated, offering pathways of healing, connection, and support. We provide spiritual care inside the Lake County Jail, creating space for cultural and personal renewal even in difficult circumstances.

As part of the Lake County Reentry Collaborative, we are building bridges to reentry by expanding services such as support groups within the jail and Enhanced Care Management (ECM) for individuals returning to the community. Through these efforts, we strive to reduce barriers, restore dignity, and strengthen the circle of wellness for justice-involved relatives and their families.

Spiritual Support for Incarcerated Relatives

We provide spiritual support for incarcerated individuals at the Lake County Jail, offering guidance, encouragement, and connection.

 

Our work fosters healing, cultural grounding, and overall wellness, creating pathways for hope and resilience even in challenging circumstances.

Important:

This scheduling form is for use by Lake County Jail staff only.

Staff may use the link below to complete the Spiritual Support Intake Form and request a visit from a spiritual advisor.

ECM for Reentry Support

Adjusting to life after incarceration can be overwhelming, including managing health needs, securing housing, reconnecting with family, and navigating community services. Our Enhanced Care Management (ECM) program is here to walk alongside you.

Joining ECM is free for those who qualify and are covered by Medi-Cal through Partnership HealthPlan of California.

​

We proudly serve justice-involved community members in:
Lake, Mendocino, Colusa, Glenn, Butte, and Tehama Counties.

How to Join:

  • Call us at (707) 900-2121 or email ECM@knwi.org to learn more or get started.

  • Visit us at our Lakeport office, and we’ll sit down with you and go through the process together.

  • Ask your probation officer, doctor, or tribal clinic for a referral.

 

We welcome you home and are committed to supporting your journey toward healing, stability, and wellness.
