Culturally-rooted support for Native families during pregnancy through early childhood

About the Family Spirit Home Visiting Program

The Family Spirit Program is an evidence-based, culturally tailored home-visiting model developed specifically for Native communities. Through in-home or virtual visits, trained Native educators provide support to pregnant caregivers and parents of young children up to age 3, with optional services extending through age 5. Families receive guidance on parenting, child development, caregiver wellness, and building strong cultural connections.

Kno’Qoti Native Wellness, Inc. is proud to offer the Family Spirit Home Visiting Program as a natural extension of the work we’ve been doing for years, centering Native culture, community wellness, and generational healing. The Family Spirit model aligns deeply with our approach: supporting families through culturally rooted care that uplifts both parents and children.

As a Native-led organization, we don’t just offer services, we live them alongside our community. Our team understands the unique experiences of Native families because we are part of those same communities. That connection strengthens our delivery of Family Spirit, making each visit meaningful, respectful, and rooted in tradition.

We bring the Family Spirit curriculum to life by:

  • Incorporating traditional practices, language, and values into every home visit

  • Offering support from trained Native educators who reflect and understand the families they serve

  • Creating a safe space for cultural identity, healing, and empowerment

  • Blending services with our Enhanced Care Management (ECM) program for added support in navigating healthcare and social services

 

This program fosters a strong partnership that nurtures families and honors cultural heritage at every step. Together, we create a supportive environment where families can thrive and diversity is celebrated.

How to Enroll

ECM Referral Forms

There are two ways to enroll in the KNWI Family Spirit Program:

Preferred Method:

Enroll through Kno’Qoti Native Wellness, Inc.’s Enhanced Care Management (ECM) program by submitting a referral to Partnership HealthPlan of California.

This option provides families with additional wraparound support and care coordination. Visit our ECM page to learn more about getting started.

 

​Self-Referral Option:

If you do not qualify for ECM, you can still participate in the Family Spirit Program by reaching out to us directly. We welcome all Native families with young children who are seeking culturally grounded support.
